ESSENTIAL FUNCTIONS: Assessment and Planning Gathers information related to individual and family, financial, health, and emotional status in order to develop plan for addressing psychosocial barriers to health and well-being of clients to develop a client directed Service Plan outlining service. Education/Suppor t Provides education and psychosocial support on issues such as health, medical compliance, budgeting, safety, parenting and community resources according to assessed need and resulting plan. Advocacy/Services Coord. Maintains information on available community resources, makes appropriate and timely referrals for services, and provides ongoing coordination of needed services. Documentation Maintains all necessary documentation required by hospital, department and funder. Outreach In collaboration with other staff, provides information and referral to community at large as well as individual patients both in the community and in the home. REQUIREMENTS: Formal working knowledge; equivalent to an Associate's degree (2 years college); requires knowledge of a specialized field.
