Family Nurse Practitioner or Physician Assistant – Central California

Salary: $140,000 – $145,000 + Productivity Bonuses

Are you a compassionate Advanced Practice Provider seeking to make a meaningful impact in an underserved community? We are looking for a Family Nurse Practitioner or Physician Assistant to join a collaborative primary care team in California’s Central Valley .

Position Highlights:

• Provide comprehensive primary care across preventive, chronic, and acute visits
• Manage your own patient panel with continuity of care focus
• Work in a team-based model alongside physicians, nurses, and support staff
• Participate in care coordination, team huddles, and quality improvement initiatives
• Limited rotational phone triage/on-call responsibilities
• Document using SOAP notes, manage labs, referrals, and treatment plans

Qualifications:

• Graduate of an accredited Nurse Practitioner or Physician Assistant program
