Job Title: Healthcare Consultant III
Location: Remote - Must Reside in or Near Winnebago County, IL
Schedule: Monday - Friday | 8:00 AM - 5:00 PM CT | Full-Time
Overview:
We are seeking a dedicated and experienced Healthcare Consultant III to join our care coordination team. This is a remote position with regular travel required to visit members in their homes throughout Winnebago County, Illinois. The ideal candidate will bring a strong background in behavioral health or human services, excellent organizational skills, and a passion for empowering individuals through compassionate and resource-driven care.
Key Responsibilities:
Conduct comprehensive assessments of referred members to determine needs and service eligibility.
Evaluate benefits and connect members to appropriate internal and external support programs.
Develop, coordinate, and implement individualized care plans and monitor progress.
Collaborate with case managers and healthcare teams to eliminate barriers to care.
Identify and escalate quality of care concerns as needed.
Negotiate with service providers to secure necessary member resources.
Provide coaching and education to support informed healthcare decision-making.
Maintain compliance with all regulatory and accreditation standards.
Qualifications:
Bachelor’s degree in behavioral health, social work, psychology, or related human services field (required).
Non-licensed master-level clinicians are welcome to apply.
Prior experience in case management is strongly preferred.
Must be a resident of Illinois, residing in or near Winnebago County.
Ability to work independently in a home-based environment and travel locally for member visits.
Strong organizational, communication, and negotiation skills.
