Job Summary

The Geisinger Skilled Nursing Program is seeking an experienced Nurse Practitioner or Physician Assistant to rotate between Mountain Top Rehabilitation & Healthcare Center, Mountain Top, Birchwood Rehabilitation and Healthcare Center and Lakewood Rehabilitation & Healthcare Center, Nanticoke, PA.

Job Duties

This position is as a Geisinger provider in a contracted skilled nursing facility and r equires an experienced advanced practitioner, prior experience working in a skilled nursing facility, family medicine, internal medicine or ICU. The advanced practitioner (AP) provides direct (hands-on) care in skilled nursing facilities with a goal to improve quality and prevent unnecessary re-admissions to the acute care setting.

• Continuing Medical Education – 10 days and $3,000 annually

Responsibilities:

• Review reports
• Communicate with SNF staff for patient needs
• Treat patients for acute needs, admissions, discharges, etc.
• Manage extremely complex patients, see every day or at least once a week
• Approximately 12 patients a day
• Length of time per patient and number of patients is per need

Education

Master's Degree-Nurse Practitioner (Required)

Certification(s) and License(s)

Licensed Registered Nurse (Pennsylvania) - RN_State of Pennsylvania; Basic Life Support Certification - Default Issuing Body; Certified Nurse Practitioner - American Academy of Nurse Practitioners
