SUMMARY OF POSITION:\n \n \n

\n The nurse navigator role aims to help facilitate the patient journey to ensure proper navigation throughout the health care system upon discharge disposition as well as in the outpatient setting in preparation for procedural or surgical care. The role aims to support staff to assist in the coordination of care while the patient is transitioning through the healthcare continuum throughout the outpatient healthcare setting.\n

\n \n MINIMUM QUALIFICATION(S):\n \n

\n
• \n Currently licensed to practice as a registered nurse by state of employment or holds a multistate RN license through the interstate Nurse Licensure Compact\n
\n
• \n Bachelor's degree in Nursing\n
\n
• \n Minimum five (5) years experience in direct patient care\n
\n
• \n AHA BLS before end of the orientation period\n
\n
