Base pay range $71,200.00/yr - $97,100.00/yr Job Description The Medical Social Worker participates in the interdisciplinary care provided to home health patients. Responsibilities include evaluating and developing personalized care plans to address patients' emotional and social needs, supervising Social Worker Assistants, and working within company policies, healthcare standards, and regulations. Responsibilities Assess the patient's social and emotional state related to illness or injury, care needs, and response to treatment. Evaluate the patient's relationships with medical and nursing needs, financial resources, and community resources. Assist in obtaining community resources to support patient recovery. Educate patients and families on coping with social and emotional responses. Collaborate with physicians and healthcare team to develop and re-evaluate care plans. Monitor and report changes in patient condition and responses to treatment. Participate in discharge planning and care coordination activities. Supervise and evaluate Social Work Assistants to ensure quality of services. Maintain comprehensive documentation as per company and regulatory standards. Required Skills and Experience Masters or doctoral degree in social work from an accredited institution. State licensure as a Social Worker, if required by law. Valid driver's license, auto insurance, and reliable transportation. Current CPR certification. At least one year of experience in healthcare, home health, or hospice settings. Knowledge of discharge planning and community resources. Excellent communication and interpersonal skills. Fluent in English (reading, writing, speaking). Knowledge of medication administration. Organizational skills and ability to work under stress. Flexibility in work hours and travel. Maintain licensure and continuing education requirements. Good attendance record.
